There’s only one Cleanse Tag available in HeartGold/SoulSilver, and you can get it by talking to the old lady inside the old Pokémon Day Care building on Route 5, located just below Cerulean City.
Route 5 isn’t accessible until quite late in the game – you’ll need to have beaten the Elite Four once and unlocked Kanto to reach it.
Other than that, getting the Cleanse Tag couldn’t be easier.
Cleanse Tag Location (Step-by-Step)
Step 1: Start by flying to Cerulean City and heading south from the Pokémon Center to where the city meets Route 5.
Step 2: Head directly south from the point in the picture above.
Before long, you’ll see a jump-able ledge with some tall grass on the other side. Hop over it.
Step 3: On the other side of the long grass is a set of stairs, climb them and you’ll find the old Pokémon Day Care at the top of the hill.
Step 4: Head inside the house and talk to the old lady.
She’ll say something quite ominous about a spirit hovering over your shoulder and give you the Cleanse Tag to ward it off.
What Does the Cleanse Tag Do?
A Cleanse Tag will decrease the rate of wild Pokémon encounters by 33.3% as long as it’s held by the first Pokémon in your party. This even works if the Pokémon holding it has fainted, as long as they stay at the head of your party.
