You can get a Cleanse Tag from the old lady inside the only building on Route 5. This place used to be the Day Care in Pokémon Red, Blue, and Yellow.

To reach the former Day Care building, head onto Route 5 from Cerulean City’s southern exit and drop down a couple of ledges.

You can do this as soon as you reach Kanto for the first time.

Prerequisites

The only prerequisite to getting the Cleanse Tag is reaching the Kanto region.

To reach Kanto, you must:

  1. Collect all eight Johto Gym Badges.
  2. Defeat the Elite Four and Champion Lance at the Indigo Plateau.
  3. Get the S.S. Ticket from Prof. Elm.
  4. Ride on the S.S. Aqua from Olivine City to Vermilion City.

From there, keep going north; you’ll be at Cerulean City soon.

Getting the Cleanse Tag

Starting at Cerulean City’s Pokémon Center, go south.

After passing between some houses and pillars, you’ll see a ledge leading into Route 5.

Go down two ledges to find the former Day Care’s location.

Inside, an old lady will give you the Cleanse Tag to ward off evil spirits.

Cleanse Tag Uses

Giving the Cleanse Tag to one of your Pokémon halves the frequency of wild encounters as long as that Pokémon holds the item.

This is useful for exploring caves or surfing long distances without being pestered by swarms of Zubat and Tentacool, for example.
